Which Form Do You Need?

The documents below cover the paperwork required to quote, bind and maintain an equine insurance policy. Submitting complete paperwork is what determines how quickly cover can start, because a mortality policy becomes effective only once the completed application and the supporting veterinary documentation have been received and approved by the appointed underwriters.

• Application. The starting point for any new equine mortality policy, capturing your horse’s age, breed, declared use and value.
• Statement of Health. A signed statement attesting to your horse’s condition, required to bind cover where the insured value sits below the carrier’s threshold — commonly $100,000.
• Veterinary Certificate. Required in place of the Statement of Health where the insured value exceeds that threshold, and completed by your veterinarian.
• Value Substantiation. Used when you want to insure a horse for more than its purchase price. Show records, training records and similar evidence are submitted for the underwriter’s review.

Before You Submit

Any horse proposed for insurance must be clinically normal and warranted in good health, and the declared use on the application forms part of the contract — the policy does not cover an animal used for a purpose other than the one stated. Renewal applications are issued 30 to 60 days before your renewal date and must be returned before the expiration date, as grace periods are not offered. Changes to a current policy must be made in writing.
